During this consultation, a medical assistant will review your health history and complete your intake. You will then have a telehealth evaluation with a licensed provider to determine if you are a candidate for medically supervised treatment plan. 

What’s included: 

  • Medical intake and health review

  • Telehealth provider evaluation

  • Personalized plan if approved

Not all patients qualify and results are not guaranteed. Consultation fee is applied toward treatment if you proceed.
